SOPA's Healthy Athletes Provider Application

Athletes might receive referrals for follow-up care from any of the Healthy Athletes screenings and Special Olympics Pennsylvania (SOPA) is committed to assisting them find the services and resources that they require when they return to their local communities.

SOPA has created a Directory with contact information for Providers. If you know of an interested Provider, Special Olympics Pennsylvania encourages you to share the following form at https://form.jotform.us/71025976998171%20 that they can complete entirely online.

Completing the application does not require them to offer free or reduced services or add anything to their normal services. It is simply a way for us to include their contact information within our Directory.
