Posted below are two versions of the Team Pennsylvania coach application for the 2014 USA National Games. Please note that in order to be considered for a coach position with Team Pennsylvania completed applications must be submitted no later than August 15, 2013.
Both of the below versions of the 2014 National Games Coach application can be completed by typing the information into the appropriate fields or checking the appropriate boxes without changing the formatting of the document. The difference in the two documents is that one is a newer Windows version (.docx) and the other is a Windows 97-2003 version (.doc). If using the Windows 97-2003 version you will have to take two additional, easy steps to fully enable the document to be completed “online”. When you open the document you will get a Security Warning that some active content has been disabled. If you click the Options button to the right of that statement and then click enable this content and Ok, you will then be able to fill out the document completely “online”, save it and attach it to an e-mail for submission. You can also simply print and complete by hand either version of the application.
